Concept Housing Association: managing agents and SD support forms

What Concept Housing Association does, what its managing agents are responsible for, and how the SD support forms for a resident are completed and kept in Residoc.

Concept Housing Association CIC is a not-for-profit community interest company that provides supported homes for vulnerable people in England. It has offices in Birmingham and Bradford, and homes in Birmingham, Bradford, Lancashire, London and Telford. Day to day, residents in many of its homes deal with a managing agent and a support worker. This page is for those managing agents: it sets out what Concept Housing says they are responsible for, and how the SD support forms are completed and kept in Residoc.

This page is written by Residoc, an independent software company. It is not Concept Housing Association's website, and Concept Housing has not reviewed or approved it. To reach Concept Housing, use concepthousing.co.uk, call 0121 455 7333 or email customer.service@concepthousing.co.uk.

What a managing agent does in a Concept Housing home

Concept Housing tells its residents they will be in regular contact with a managing agent. According to its guide for residents, the managing agent is responsible for:

  • the standard of the accommodation
  • helping the resident apply for Housing Benefit
  • arranging time with the resident's support worker
  • helping the resident create their support plan
  • responding to the resident's concerns

Each resident also has a support worker, who writes a personal support plan with them, visits regularly and gives advice and guidance.

The SD forms in Residoc

Concept Housing's website describes these duties, but it does not publish a list of forms. The list below is Residoc's own: the 20 SD form templates Residoc gives a resident in a Concept Housing home, with the numbers and names Residoc uses. The review dates are how Residoc books them.

Providers should confirm the current forms, review schedules and procedures that apply to their service with Concept Housing Association.

An SD-13 Support Session Record, step by step

This is an SD-13 being completed in Residoc. The support worker answers one short question at a time, in their own words. The record builds on the right as they go.

An SD-13 Support Session Record in Residoc. The conversation with the support worker is on the left and the record it produces is on the right.
An SD-13 part-way through. The worker's own words are on the left. The record Residoc writes from them is on the right.

Filled in before the first question. The resident's name and address, the date of the session and the support worker's name.

What the worker gives. How the support was delivered, whether anything was followed up from the last session, the support plan goal the session worked on, a summary of the support, how long the session took, and whether the resident was happy with it. Photos can be added as evidence.

What the worker checks before signing. Every answer is listed beside the conversation. The worker can change any of them, and a reworded answer is shown next to their own words so they choose which one is kept. The resident and the worker then sign on screen, and Residoc files the finished PDF to that resident.

How Residoc helps a managing agent

Residoc is supported housing software. Support workers complete each SD form as a short guided conversation, typed or spoken, and Residoc produces the finished PDF, signed on screen and filed to the right resident.

  • Details given once. A resident's National Insurance number, date of birth and contact details from the SD-1 are filled into the later forms that ask for them.
  • Reviews booked for you. Finishing an SD-11 books its four SD-12 reviews, and finishing an SD-9 books its risk reviews. Each appears in the calendar.
  • See due and overdue paperwork. Residoc shows each worker what is due and overdue, and shows the provider every resident's outstanding paperwork in one list.
  • A resident's file in one download. Every record for a resident, or the move-in forms plus everything dated in a period you choose, comes out as one ZIP file. What an auditor asks for depends on the audit.
  • A letter suggested after a missed appointment. When a worker marks an appointment missed, Residoc suggests the SD-16, SD-17 or SD-18 template that fits how many have been missed. Staff decide whether to start the letter, and review it before it is used. Residoc sends nothing itself.
  • Paper files brought across. Existing residents' paper SD forms are copied in one part of one form at a time.

Managing agents can try Residoc for free on their own residents.

No. Residoc is an independent software company, and Concept Housing Association has not approved or endorsed it. Residoc makes software that managing agents and support workers use to complete and keep SD support forms.

Is Concept Housing Association taking on new providers?

Check directly with Concept Housing. When we checked on 13 October 2026, its home page said it was looking for providers who want to grow with it, while its partnership page said it was not accepting applications from new providers at the moment. Email businessdev@concepthousing.co.uk to confirm the current position.

What paperwork does a managing agent keep for a Concept Housing resident?

Concept Housing's guide for residents says the managing agent looks after the standard of the accommodation, Housing Benefit, time with the support worker, the support plan and the resident's concerns. In Residoc, those are recorded on SD forms such as the needs assessment, licence agreement, risk assessment, support plan and reviews, support session records and contact log. Confirm the forms your service must keep with Concept Housing.

Can the SD forms be completed on a phone?

Yes, in Residoc. A support worker answers one question at a time on a phone, tablet or laptop, in the resident's home, and the resident signs on screen.
