Support session

5 October 2026 · 10:00–10:45

Alex chose three meals for the coming week. We worked through the ingredients together and made a shopping list.

Alex said that choosing meals in advance made the task feel more manageable. We agreed to review the list at the next session.

From conversation to documentation

The right form.
Your account of the work.

Every session is recorded against the right resident and the right form, so nothing ends up in the wrong place and nothing is left blank.

Choose the resident and form

Pick who the session was with and the document you need. Residoc knows exactly which questions each form needs, from a session log to a contact log, a support plan or a review.

Answer in your own words

Type or speak your answers as you would explain the session to a colleague. Residoc puts each answer in the right field and keeps asking until nothing is missing.

Sign and it’s filed

Check the finished record and sign it on screen. It’s saved as a PDF in the resident’s file, ready for a manager’s review or an inspection.

From note to record

Say it once.
Every field filled.

One short note from the worker becomes a complete, signed Support Session Record. Every required field is covered, so there are no gaps for a manager or an inspector to find.

A record worth reading

Make the next reader
understand the work.

The best record isn’t the longest. It’s the one a manager, an inspector or the next worker can understand at a glance.

What the resident said

Their own words, kept separate from the worker’s view, so every record carries the resident’s voice.

What you actually did

The support actually given, in specifics. Residoc asks for the detail, so no record just says “support given”.

What happens next

The next step that was agreed, so the following session starts exactly where this one left off.

The details

A few things worth knowing.

The details behind the day-to-day work.

Will every session record be complete?

Yes. Residoc asks for every field the form needs and keeps asking until nothing is missing, so every record is complete and consistent, whoever writes it.

Does Residoc write the record for my staff?

Your staff answer in their own words, typed or spoken, and Residoc puts each answer in the right place on the form. They check the finished record and sign it before it’s saved.

Where do finished session records go?

Straight into the resident’s file as a signed PDF, with the date and the worker who completed it, ready for a manager’s review or an inspection.

Can I see which sessions have been recorded?

Yes. Providers see completed records across the whole service, and any forms left unfinished, so gaps are spotted early.
